What we do for your patients.

Now accepting referrals for patients with major depressive disorder and treatment-resistant depression.

01.

We will work with the indemnifiers on your patient’s behalf. (And, we’ve gotten quite good at it!)

02.

You will send your most recent chart notes and medication lists to us, then, once cleared, your patient will have an intake appointment with our PMHNP. Often times, this is via telehealth.

03.

Treatment consists of twice-weekly visits to Evolve Health, followed by weekly visits. Eventually, we will be able to start spacing out your patient’s treatments.

04.

Your patients will continue to see you regularly and they will continue to take their oral medications.

05.

You will receive quarterly written updates regarding your patient’s progress.

Spravato and ketamine have been shown to be effective in treating depression when oral antidepressants are not.

Evolve Health is made up of a group of experienced and compassionate medical providers. Our clinic is comfortable for patients; however, behind the scenes, we have technologically advanced medical equipment, continuous vital signs, video monitoring of all patients, with full medical support, including emergency medical equipment and medications.

Evolve Health is local. You can pick up the phone and call our providers or email them directly. We will collaborate with you on a regular basis, which includes regular written updates on all your patients.

We have a special privacy-protected protocol so that the medical providers in our community can also get the care they need.
